PERMISSION TO USE IMAGE
I authorise RSPCA Qld to use my image(s) (in a photograph or video footage) in print and electronic media including publication on their website.
My image(s) may be displayed on bulletin boards, used in promotional material or in video footage, or shown to current and prospective clients, at any time now or in the future.
I understand that my name may or may not be used with my image(s). This permission is given with no promise or expectation of value in return.

Terms and Conditions: Subject to availability at all times. Economy Class Travel. Travel must be booked by 31 March 2015 and completed by 30 June 2015. Blocked out for travel between 10 December 2014 until 20 January 2015 along with all Australian Easter school holiday break and special event periods. Prizes are non-refundable, exchangeable or redeemable for cash. Travel requests must be booked at least 30 days in advance and prizewinner is responsible for any amendment fees or additions costs charged by the airline due to any amendments or cancellation of travel once reservations have been confirmed.
My RSPCA Pet Testimonial Campaign
- The RSPCA QLD My RSPCA Pet Campaign Competition (“the Competition”) is run and operated by RSPCA QLD, of, Wacol, Qld, 4076.
- The following persons are ineligible to enter the Competition:
- Employees of the RSPCA
- Relatives of Employees of the RSPCA
- Promotions in relation to the Competition will commence on Monday the 24th of November 2014 and run until 11.55pm Queensland time on the 31st December 2014 (“the Promotion Period”). Entries submitted or received after the closing date will be deemed invalid and will not be accepted.
- The winners of the 10 prizes, a Fujifilm Instax Mini 8 Instant Camera (colours may vary) and a 10 pack of Fujifilm Instax Mini Film) will have submitted a valid testimonial, including a photo or video of their RSPCA pet, as part of the campaign.
- Names and likenesses of Promotion and Competition winner(s) may be used by the RSPCA for promotional purposes without the need for further consent and without further compensation to the Promotion and Competition winner(s).
- A valid entry constitutes a completed testimonial submission via www.myrspcapet.com.au. Any entry form that does not adhere to term 4, will be deemed invalid.
- After the closing date, all submissions will be held on file at the RSPCA QLD Brisbane Animal Care Campus, 139 Wacol Station Road, Wacol, QLD, 4076 until the announcement of the Competition winner(s).
- The winning entry(s) will be announced by RSPCA Qld on Monday, the 5th of January, 2015 and the holder of any winning entry will be notified on this date.
- In the event of fire, theft, or irreparable damage to the entry barrel and/or entry forms, the RSPCA retains the right to suspend or cancel the Competition.
- Members are not permitted to fill in a testimonial submission on behalf of any other persons. Submissions are not transferable.
- Conduct of the Competition, including decision on the eligibility of persons to enter, shall be the sole responsibility of the management of the RSPCA, whose decision in all matters shall be final. No correspondence shall be entered into.
- The RSPCA reserves the right to disqualify any entry that is deemed fraudulent or not a valid entry form as issued by it.
